Bdnf Abnormalities May Predict Bipolar Disorder

Numata and team also conducted a meta-analysis of six studies assessing the association between the MTHFR C677T polymorphism and schizophrenia in Japanese populations (4316 schizophrenia cases and 6062 controls). They found that carriers of the CC genotype had a 16% increased risk of schizophrenia compared with carriers of the TT genotype. There was no significant heterogeneity among the studies. When the researchers used Mendelian randomisation to combine the results of the genetic association meta-analysis with those of the gene –homocysteine association study, they found that each standard deviation increase in plasma homocysteine level was associated with a 14% increased risk of schizophrenia . Writing in Schizophrenia Bulletin, Numata and co-authors conclude that their study suggests that “increased plasma total homocysteine levels may be associated with an increased risk of developing schizophrenia”.

In The Second Cohort (karolinska; 215 Mood-stabilised Patients With Bipolar Disorder And 112 Controls), The Sensitivity Was 74% And The Specificity Was 64%.

A statistical model that included all four serum markers discriminated between patients and controls in the Sahlgrenska cohort with a sensitivity of 89% and a specificity of 77%. In the second cohort (Karolinska; 215 mood-stabilised patients with bipolar disorder and 112 controls), the sensitivity was 74% and the specificity was 64%. When the researchers added Global Assessment of Function (GAF) scores to the model, sensitivity in the Sahlgrenska cohort increased to 100% while specificity increased to 95%. This “would be strong enough to work as a clinical biomarker predicting the diagnostic dichotomy,” they remark. GAF data were not available for the Karolinska cohort. Of note, the team found no significant differences in serum proBDNF, mature BDF, the proBDNF/mature BDNF ratio or MMP-9 levels among patients with bipolar I, bipolar II and bipolar not otherwise specified. There was also no correlation between the serum markers and Montgomery-Åsberg Depression Rating Scale and Young Mania Rating Scale scores. Writing in the Journal of Affective Disorders, Kristoffer Södersten (University of Gothenburg, Sweden) and co-authors conclude: “Our results indicate that BDNF measurements have a potential for usage as clinical biomarkers by differentiating bipolar patients from healthy control individuals.

Tooth extraction before heart surgery carries risks

The mean age of patients was 62 years; 80% were men. The median time between tooth removal and cardiac surgery was 7 days. “The prevalence of major adverse events should advise physicians to evaluate the individualized risk of anesthesia and surgical procedures in this population,” the researchers suggest in TheAnnals of Thoracic Surgery. Although they cannot give a definitive recommendation for or against dental extraction based on their findings, the researchers hope that the results will spark discussion and encourage others to carry out future research. “In the meantime, we recommend an individualized approach for these patients, to weigh their particular risk and benefit of a dental procedure before cardiac surgery with the information we have currently available,” Grim advised.
